Preparation for Patients
When you visit Genetics Center, we want you to feel as prepared and comfortable as possible for your appointment. Please keep the following considerations in mind before your visit to Genetics Center.
What to Bring
Before your appointment, please fill out and print the Patient Registration Form for Genetic Counseling. Complete the questionnaire and provide your insurance information prior to your appointment. Please bring your Patient Registration Form and your Insurance Card on the day of your appointment.
Other Considerations
A full bladder or fasting is not necessary for genetic testing procedures and ultrasounds. If you are bringing a child who is younger than 13 to a genetic consultation or ultrasound appointment, please have another adult there to watch your child during the appointment, or you may be asked to reschedule. Photography, audio and/or video recording are not allowed at Genetics Center facilities.
